Make plans to send your 3rd through 5th grader with us! WHEN: July 5-July 9 (Meet at PVBC at 9am on July 5th. Pick up students at PVBC mid to late afternoon July 9th.) WHERE: Jenness Park COST: $340 PACKING LIST BELOW PARENT MEETINGS (choose one) Sunday, June 5th @ 12:30pm Monday, June 6th @ 7pm Get your forms notarized! We'll have info and refreshments for you! STEPS TO GO TO CAMP: 1.Fill out online form at bit.ly/registercentrikid 2. Print and fill out the CentriKid form at bit.ly/centrikidform 3. Have each CentriKid Form notarized (IMPORTANT) 5. Print and fill out both pages of the PVBC waiver at bit.ly/pvbcform (Does NOT need to be notarized.) 6. Make a copy of student’s medical insurance card 7. Cost is $330 per student. If you need a partial or full scholarship, please let Pastor Michael know. Write check for full amount (or remaining balance after scholarship) to PVBC. 8.
